HOA Forms

The Homeowners’ Association has provided the following documents to download for your convenience. In order to download the documents you will need to have Adobe Acrobat Reader installed on you computer. If you do not have Adobe Reader you can download it by clicking on this link.

Architectural Control Committee Application Forms:

• ACC FORM A: Exterior Painting. Use this form for all exterior painting of the house.
• ACC Form B: External Alterations. Use this form for the following:
  • Windows
  • Doors – including garage doors
  • Additional outdoor structures

• ACC Form C: Landscape. Use this form for removal of or changes to front yard landscaping. Remember to use only approved plants.

• ACC Form D: Pool, Hot Tub, Spa. Use this form for installation of new pool, hot tub and or spa. This form requires an $800.00 non-refundable water payment.

• ACC Form E:  Over 25% of Roof: Use this form for repairs to 25% or more of the roof. This includes applications of sealers and paint.

• ACC Form F:  Completion Notice: Use this form when your project is finished. The ACC Committee will inspect and approve return of your deposit from MMI.

• LVN Remediation Application: Use this form for replacing your lawn due to LVN.
